This refund policy applies to subscriptions purchased through:

Our refund policy does not affect your statutory rights.
1. CANCELLATION & REFUND POLICY
If you want to cancel your Bliv Wellness LLC subscription within 72 hours of your purchase, you may request a full refund for the amount you paid using the instructions below.
If you want to cancel more than 72 hours of your purchase, but before your initial practitioner appointment, you may request a refund for the amount you paid, less a $200 fee to cover your account set up and our initial clinical coordination activities. If you have paid with CareCredit, the 15.99% service fee will not be refunded. If you fail to attend your initial doctor’s appointment, you are not entitled to a refund. The cost of shipped medication will not be refunded under any circumstance.
During treatment, your Bliv Wellness LLC provider determines it is no longer appropriate for you to be on a GLP-1 medication. There must be official clinical documentation articulating the medication adverse side effects or conditions requiring the discontinuation of medication. There must be an attempt to switch to another GLP-1 medication or weight loss solution before a refund will be considered. In this scenario, you are eligible to update your Membership to our health coaching package, accepting the $49.99 monthly payment. Alternatively, you may cancel your Bliv Wellness LLC Package and we will reimburse your unused Package Fees less any incurred Program Costs, rounded to the nearest month (e.g., if you have already made a Package Fee payment covering the next three months, but treatment will discontinue in the first month of that subscription term, you may request a partial refund for the amount reflecting the second and third month of your Package Fee, minus any Program Costs incurred by Bliv Wellness LLC, which include medical consultations, insurance and benefits verification, prior authorization management, and lab work (if applicable)).
You are clinically eligible for a branded GLP-1 medication, insurance does not cover the cost of GLP-1 medication and/or coverage requires a copay >$500 per month, and you are ineligible for alternative medications. In this scenario, you are eligible for a refund of the Package Fees you were charged minus the cost of Program Costs incurred by Bliv Wellness LLC, which include medical consultations ($200), insurance and benefits verification, prior authorization management ($75), and lab work (TBD) (if applicable).
